At SurgiCore Ambulatory Surgery Center (“SurgiCore,” “we,” “our,” or “us”), we are committed to conducting all aspects of billing and reimbursement with honesty, integrity, and full compliance with federal and state laws and payer requirements. Our professional billing compliance program is a key component of our broader corporate compliance efforts.

Purpose and Scope

This policy applies to all SurgiCore employees, affiliated physicians, billing personnel, contractors, and third-party vendors involved in the documentation, coding, and submission of claims for medical services provided at SurgiCore.

Core Principles

SurgiCore upholds the following professional billing compliance principles:

• Accuracy in medical coding, charge capture, and claim submission
• Compliance with all applicable federal and state regulations, including the False Claims Act, Anti-Kickback Statute, and Centers for Medicare Medicaid Services (CMS) guidelines

Documentation and Coding Standards

We require that:

• Medical records are complete, legible, and accurately reflect services rendered
• Coding is based on clinically appropriate documentation
• All claims reflect the actual services provided, without exaggeration or omission
• Upcoding, unbundling, and duplicate billing are strictly prohibited

Education and Training

All billing personnel and providers receive regular training in proper documentation, coding, billing practices, and updates to regulatory requirements. Ongoing education ensures that all parties understand their responsibilities and stay current on applicable laws and standards.

Internal Audits and Monitoring

SurgiCore conducts periodic internal audits and reviews of billing records and claims to:

Reporting Concerns

Anyone who suspects unethical billing behavior or potential noncompliance is encouraged to report their concerns. SurgiCore maintains a strict non- retaliation policy for good faith reporting.
